To prepare for your first visit, please:

Please ensure that your intake paperwork has been completed.  Generally you will have received this via email.  Please complete the intake paperwork within 72 hours of scheduling the appointment.  If your paperwork is not is essential that your new intake paperwork is completed within 72 hours completed within this timeframe it is  possible that your appointment may be cancelled. 

Complete the appropriate forms below and bring them to your first appointment.

Please bring your insurance card and a Government-issued form of identification, such as a birth certificate, drivers license, or a DMV identification card.
